Understanding Feline Urinary Issues: When to Worry and How to Help

If your cat is visiting the litter box frequently, straining to urinate, or crying in discomfort, it may be more than “just a UTI.” Feline urinary problems are common and can range from mild irritation to serious medical emergencies.

At Pet Menders Animal Hospital in Boulder and Frederick, Colorado, our veterinarians frequently treat urinary conditions such as urinary tract infections (UTIs), bladder stones, and Feline Idiopathic Cystitis (also known as Pandora Syndrome).

First, a brief synopsis of the most common conditions:

Urinary Tract Infections (UTIs) occur when bacteria invade the bladder, leading to inflammation and painful urination. While UTIs are more common in older female cats, they can occur in any cat. Symptoms include frequent urination, blood in the urine, and accidents outside the litter box. A simple urinalysis helps identify infection so that the right antibiotic and treatment plan can begin quickly.

Urinary stones (also called uroliths) are mineral formations that can irritate the bladder or block the urethra—especially in male cats. Blockages are life-threatening and require immediate veterinary care. Depending on the stone type, treatment may include prescription diets or surgical removal.

Pandora Syndrome describes cats with recurring urinary issues not linked to infection or stones. These cats often have sensitive stress responses, so management includes both medical care and environmental enrichment—like providing hiding spots, playtime, and consistent routines.  This too can lead to a urinary blockage in male cats that can be a medical emergency.

So what is a Pet Owner to do?

First, if your cat is urinating outside of the litter box, bring the cat in and we will usually start with a urinalysis.  This will help determine the cause. If this is inconclusive the next steps may be x-rays, an ultrasound or blood work. Because urinary symptoms can signal anything from mild inflammation to a dangerous blockage, it is best not to wait…..not to mention your carpet and baseboards will appreciate you 🙂
